ORAL AND DENTAL HEALTH APPLICATION AND RESEARCH CENTER CLARIFICATION TEXT ON THE PROCESSING OF PERSONAL DATA OF PATIENTS

This clarification text is published by the Oral and Dental Health Application and Research Center of Istanbul Kent University, as the data controller, in accordance with the Personal Data Protection Law No. 6698, the Regulation on Personal Health Data, the Communication on the Procedures and Principles to be Followed in Fulfilling the Disclosure Obligation and the relevant legislation. It has been prepared for use in its activities.

HOW DO WE PROCESS YOUR PERSONAL DATA?

Patient Registration

When you enter Kent University Dental Hospital as a patient, you must first register with your identity number. This personal data is instantly reported to the General Directorate of Security through the automatic system. If the system verifies, your name, surname, date and place of birth, parents' names and address information are automatically transferred to the Hospital Information Management System where we receive your registration.

Following this process, your following data will be processed by our staff in order to complete your registration in the hospital information management system, based on the legal reasons "provided that it is directly related to the establishment or execution of a contract, it is necessary to process the personal data of the parties to the contract" and "it is mandatory for the data controller to fulfill its legal obligation" is processed.

As a result of these procedures, a "Patient Code" is given to all our guests.
All these data are instantly transferred to the Medula system of the Social Security Institution through the Hospital Information Management System, where they are processed.

Health Care

For our patients whose registration process is completed, the following data will be processed and recorded in the Hospital Information Management System by our healthcare personnel and Dentists, who are under the obligation of confidentiality, during the provision of the health service they request and due to the nature of this service, especially for the legal reason "conducting medical diagnosis, treatment and care services".

Health Data (including, but not limited to, obtained as a result of medical diagnosis, treatment and care services) All these data are held by the Republic of Turkey. It is instantly transferred to the E-Pulse system affiliated with the Ministry of Health.

After our patients' medical services are completed, for the purposes of collecting the medical service fee and issuing invoices according to the relevant insurance status and "provided that it is directly related to the establishment or execution of a contract, it is necessary to process personal data of the parties to the contract" and "it is mandatory for the data controller to fulfill its legal obligation." At Kent University Dental Hospital, monitoring and recording are carried out with a closed circuit camera system. This process is automatically based on the "reason for processing the data for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the relevant person", in order to ensure the physical use security of all visitors to our center and the security of visual and audio recorded data controller operations, as well as the management and tracking of visitor records. will be processed.

HOW DO WE TRANSFER YOUR PERSONAL DATA?

Health Care

Your personal data listed above may be notified to authorized institutions and organizations by persons appointed by competent authorities, especially but not limited to the Ministry, Provincial and District Health Directorates, Public Health Centers and other units affiliated to the Ministry, or within the scope of established Medula, Electronic Pulse and similar automatic systems. and/or within the scope of our reporting obligation, to domestic and international laboratories, ambulances, medical devices and institutions providing health services with which we cooperate for medical diagnosis and treatment, through our subsidiaries and/or affiliates and/or through the common Hospital Information Management System used by all our hospitals. To our group companies, to the relevant health institution when you need to be referred, to the relevant health institution abroad if you request a second opinion from a physician abroad, and to the legal representatives you have authorized, for the purpose of providing health services and "protection of public health, preventive medicine, medical diagnosis, treatment and care services" execution, planning and management of health services and financing” will be transferred based on the legal reason.
In addition, your personal data, limited to the identity number of the Republic of Turkey, will be transferred to the General Directorate of Security based on the legal reason that "it is mandatory for the data controller to fulfill its legal obligation" in order to provide information to authorized public institutions and organizations.

Services Received from Third Parties

Our subcontractors, business partners, legal representatives that we receive consultancy from, authorized by us and third parties, domestic/foreign organizations and other third parties and legal representatives from whom we contractually receive service to carry out our activities, with whom we cooperate, and only in connection with the service received, in a limited and measured manner. and on the condition that commitments are taken stating that they will take all technical and administrative measures required by the Personal Data Protection Authority throughout the service.

Insurance Service

Regarding the private health services offered to guests within the scope of the Social Security Institution at Kent University Dental Hospital and in this context, for the purpose of benefiting from complementary health insurance or private health insurance, the name, surname and mobile phone number of the relevant person are processed for the provision of medical diagnosis and treatment. The data is transferred to the consultant and insurance brokerage company with whom a cooperation agreement has been made, based on the explicit consent to be requested separately in this regard.

Educational Institutions We Collaborate With
